TidalHealth and Somerset & Wicomico County Health Departments are conducting their triannual Community Health Needs Assessment (CHNA) in accordance with the federal requirements of the Affordable Care Act.

Healthcare leaders in our region can better understand needs by gathering many different voices from the community. This survey allows community members to share with those responsible for the delivery of healthcare services what they feel are the important issues affecting Somerset, Wicomico, Worcester and Sussex counties.

The results of this survey, combined with other community data, will help with collaborative efforts between TidalHealth and the local health departments to better understand community health concerns, and to guide improvement efforts for adults, children and adolescents.
